Your Reservation Request Form must be received by Memorable Destinations within twenty-one (21) days from the postmark on the front of the mailing envelope.
Fill out the Reservation Request Form.
Choose your three (3) check-in options for hotel stays (in the order of preference)
Return by certified mail both your refundable reservation deposit in the form of a check or money order for $129.00 (In the event of a returned check there will be a $25 fee.) In addition, your pending reservation request will be cancelled. Send your Reservation Request Form to:

Your deposit and this original Reservation Request Form must be sent to Destinations Unlimited, LLC. by certified mail.

Three (3) dates required for processing or your request will not be considered. I have read and agree to all of the qualifications, terms, conditions, and procedures of this certificate.

This contract is for the purchase of a vacation certificate and puts all assignees on notice of the consumer's right to cancel under section 559.933, Florida Statutes.

Qualifications, Terms and Conditions

IIn order to participate in this offer, you must be at least 21 years of age and be a U.S. resident. Married participants must use the Memorable Destinations certificate together.

Eligible participants will receive 3 day / 2 night hotel accommodations for two adults at participating properties. Participating hotels will not accommodate any pets.

You must register this certificate by submitting your reservation request form as described herein. When requesting your reservation, you must choose 3 arrival dates (this is required to process your reservation due to changing availability) and must provide a minimum of 21 days advance notice. When submitting your reservation request, a $129 refundable room deposit will be required and must be sent, along with your reservation request form, via certified mail. Your activation date will be the date your certified letter containing your reservation request form is postmarked. Your stay must be completed within 90 days of your activation date. Once your deposit and completed reservation request form are received at Destinations Unlimited, LLC. your request will be processed. Destinations Unlimited, LLC. will then contact you to confirm your registration date and check-in date.

After we have confirmed your reservation over the phone, Destinations Unlimited, LLC. will prepare and mail a written confirmation with directions and check-in times. This correspondence will be issued approximately 21 days prior to your arrival date.

At the time of check-in, you will be required to present two forms of identification, including your driver’s license and a major credit card and your written confirmation.

This offer is for complimentary accommodations only. It does not include any other travel-related expenses, such as food, transportation, taxes, tips, telephone calls, energy surcharges, or items of a personal nature. Service fees for certain amenities may apply.

This offer is good for complimentary accommodations at participating hotels only and is based on availability. Check-in days are Monday and Tuesday only. Destinations Unlimited, LLC. reserves the right to decline reservation requests during peak seasons, conventions and holidays.

Cancellation policy: There is a $50 non-refundable fee should you reschedule or cancel a confirmed reservation, except where prohibited by law. If you cancel a confirmed reservation, Destinations Unlimited, LLC. requires a written cancellation notice, which must be received by Destinations Unlimited, LLC. at least seven days prior to your arrival date. If you fail to check-in pursuant to a confirmed reservation, or if you fail to cancel seven days or less prior to check-in, you will forfeit your $129 deposit.

Upon completion of your Destination Las Vegas getaway, you may request a refund of your deposit by mailing a copy of your reservation letter, your hotel check out statement, and a self addressed, stamped envelope to: Destinations Unlimited, LLC. 7370 Dean Martin Drive Suite 409, Las Vegas, NV 89139.

This offer is void where prohibited by law. All federal, state, or local laws apply. Destinations Unlimited, LLC. assumes no responsibility for any verbal or written representations made in conjunction with this offer by any employee of Destinations Unlimited, LLC. other than those included in these Qualifications, Terms and Conditions.

This certificate may not be used concurrently with similar certificates, and may not be combined with any other offer or discount. Two or more vacation certificates may not be used within the same year. This offer is non transferable.

This certificate cannot be sold or exchanged for cash and no refunds will be issued for any unused portion of this vacation offer. The retail value of this offer varies depending upon the time of year and hotel. This offer is not for retail sale.
